3rd attempt with the same scores BPs in all 5 domains
I don't know what else to do at this point. I took 2 prep courses read Headfirst, Kim Heldman, Rita's 7th edition book and study really hard over the past 6 weeks, and still couldn't pass it. And, yes I did the mock exams my scores range from 50s to mid 70s in that order. I've taken the following mock exams Rita's Fast Track, Simpilearn, Brainbok , PMTI, and PMPforsure.
I know I'll have to wait year per PMI's rule. This is so crazy. I actually felt confident in taking the exam today. I also hold a CAPM as well that will expire in 2014. I got all BPs for the 3rd time. What am I doing wrong?
Please help me!

Can you give us a detailed
Can you give us a detailed account of your study methods?
Do you take notes? Did you try a brain dump? What specifically do you have trouble with during the exam?

I did it all
Yes, I did the brain dump. I took notes, read books, mock exams studied for hours at a time and did not get one domain-this is so crazy to me. I can not figure out where I went wrong each time. Things should've got better for me, instead it stayed the same. I actually cried today. The only thing I can think of is that I rushed into this trying to beat the deadline of the new exam starting today. I started studying at the beginning of June and I guess I didn't give myself enough time to learn it. Rita's book talks about this a lot not being experience enough and trying to memorize the different processes because it doesn't work.
I'm not new to PMI I currently hold a CAPM cert, but of course that doesn't carry any weight like the PMP cert. I just can't figure this one out.

The only brain dump I did
The only brain dump I did before my exam was to write page 43 of PMBOK, just to follow others, I remember this helped me answering one question that asked which group/process this belongs to. Also, it was a tough task of writing the process chart using a pencil.
I started my preparation in Nov and had exam in July 26th. I rad Rita 5 times, PMBOK 4 times. Towards the end PMBOK didn't feel that boring but Rita did. There are things in PMBOK that Rita didn't cover. I attended at least 3000+ questions. The 1000 Scordo questions alone took a month to finish. I got soft copies all books, took print outs by chapter. Every time went out I had one of thos eprinted chapters in my hand. I had page 43 posted in walls in my house. I printed most of the mock tests so that i can take those tests any odd time. I can go through 15 question over the dinner table, 30 qns over tea etc.
I have the worst memory, I have anxiety troubles for which I take medicines. So, I knew clearing a PMP exam isn't an easy task for me. Evaluating my strength and weekness I decided never to go for exam until I am sure I am prepared. I planned to study as much as I can and to be in a state where I could deal with all types of question even if it takes a year. To overcome my anxiety troubles I booked the exam only 5 days before. Day before exam I didn't rush at all. I had a question, if I still fail in the exam is PMP worth studying more, and I had the answer too, with this much of preparation I would never fail. After I crossed 200 qns came the survey, I maked all answers and before going for the final screen took a moment to decide my next plan what if I get fail message - let the heart bit slowed a bit else I would have an attack - I decided I would still buy the tablet which i planned to buy for my daughter if I clear the exam, and I would buy new books start preparation and re-appear in December. I got the courage, and pressed the button. I cried too seeing the result. I cried all the way out of the building. The admin were confused at me looking at my result report. I still got 4MP, 1P which I didn't care about. But, all I can say, if I can do it anybody can do it as long as you are determined to do it. I understand emotions of all who couldn't make it but all I can say- start over, start fresh, it's not impossible.

Keep your head up!
I had to take the exam a couple times too. I was very discouraged but then found a way to study that worked for me. Instead of traditionally reading the chapters 1-2-3-4-5, etc. I created a chart of the processes and begin studying only the sections of the knowledge area that applied to that process. I wouldn't move on to another process unless I had the previous one mastered. In doing this, not only was I able to retain the information, but I was able to effectively apply it. I hope this helps.
Don't loose faith!
