Defect - Repair - Is change request or claim request ?

Zillians,

When I was reading 4.3 p 85/116 PMBOK --- App change request - as input to Direct and manage project execution process, I found it includes as app'ed change requests - PMP, Policies, Scopes, etc and also Corrective actions and Preventive actions but does not mention Defect repair.

While in Output it includes Defect Repair, as change request.

KK and I was on the debate , is Defect - Repair would be "Change" request or "Claim", because it is not any change, in scope at all.

And concluded yes for internally it will be a change request , because it impacts on schedule and cost.

While externally it is cost of failure or for customer it is Claim.

Then question remained unanswered - why it is not included as appd change request as input to execution? I f not here then where and in what form it will be attained.

Should we satisfied only on the Dot notes of INPUT -4 OPA - no 3 and 6 p86/117

Hope ANAND / and other prominents could throw light on this issue.
