e.g. project management plan is not an output of the quality assurance. Project Management plan updates might be output of the QA, but not the full plan.

Respected Pawarji Sir,
Wherever it is from PMBOK-3 or 4 but the bottom line is the answer is absolutely INCORRECT.
I am surprised that prodcast is sullying their image by posting such questions and confusing many. I know a bit of ITTOs, like you respected Pawarji Sir, so we could understand and pinpoint but imagine what would happen to beginners.
The real (PMBOK-4) outputs of Perform QA, which is an executing process, are:
A. PMP Updates
B. OPA Updates
C. Proj Doc Updates, and
D. Change Requests.
Last but not the least respected Pawarji Sir, if it is output then it has to be an Update, unless in Initiation process. Ex, what is the update of Develop PMP or Develop PC, the answer would be PMP and PC respectively and not PMP updates or PC updates.
PMP is not used in any planning process BUT is used on all processes in EPG or M&CPG. In short, PMP is not used in Developing HR but is used in all HR processes like Acquire, Develop and Manage Project Team.

Exception...
I missed one thing to mention.
PMP is not used for any Planning process, but there is one exception.
PMP updates are used for Plan Risk Responses (Planning) in spite of PMP not used an input to Plan Risk Responses. SO, we can say PMP is surely not used as an input to any planning process. Exception being the updates to Plan Risk Responses.
EEF updates are used an output to just TWO processes and they are:
(A) Develop PT, and
(B) Manage PT.
OPA is used in Close Project/Phase but not in Closed procurements. PMP takes over in Closed Procurements.
