Question clarification needed on Risk and Scope

Found this question in internet, the answer given in the site is (a). I feel the answer should be (d). Please correct me, if my understanding is wrong. My understanding is , planned risk responses are updatedin the risk register and a PM should look the risk register for risk responses. Appreciate your help!
- Beta is the Project Manager of a Road construction project. During a project review, Beta realizes that one particular risk has occured. To take appropriate action against risk that has happened, Beta needs to refer to which document?
- Risk response plan
- Risk management plan
- Risk breakdown structure
- Risk register
Similiarly, for the below question I feel that the answer should be (d), but the answer given in the site is (c).
- Completion of a product scope is measured against:
- Product Management Plan
- Project Management Plan
- WBS & WBS Dictionary
- Product Requirements

I agree with you for both questions. ONly thing I can advise here take questions from some other sources. ONe of the biggest learnings to apply to keep yourself restricted to few reputed practice test sources. Since this will not only save your time but some of these tests can lead you in unwanted direction.

For the first question - Risk
For the first question -
Risk register contains the details of the risk that were identified during the identify risk process(11.2) while risk response contains the mitigation plan/approach. Since the PM has already realizes the occurrence of a particular risk and needs to take the action, he/she should refer risk response instead of risk register.
Second question -
Process of requirement gathering (Project scope management) as per the PMBOK includes following activities
1. Collect requirements
2. Define scope
3. Create WBS & WBS dictionary.
Project/Product scope is defined in terms of WBS and all of the collected requirements may not be the part of the final scope.
