Outputs of process quality assurance

Some of those outputs are:
- CR: "increase effectiveness of the policies, processes, and procedures of the performing organization." (source PMBOK)
- Project management plan updates
- Project document updates:
- Organizational process assets updates.
1) I have read that approved CR are not implemented by this process but by the direct and manage execution process. In this case, why are the project management plan, project documents an output of the perform quality assurance process?
2) Organizational process assets updates do not need a CR to be modified, right?

Quality Policy
"If the performing organization lacks a formal quality policy,........., the project management team will need to develop a quality policy for the porject" (PMBOK)
If this scenario is applicable to the project under consideration, then a CR seem to be a valid o/p of the Perform Quality Assurance process. This in turn could nesseciate various other documents/plan to be updated.

As a result of CR , there can
As a result of CR , there can be changes to project schedule, cost, Qualtiy , risks , procurement etc. Hence PM Plan is updated to reflect the changes as a result of CR>
