When to set contingency reserves?

Dear All,
In what process do we plan the contingencay reseves?
According to Rita, in Quantitative Risk analysis we set the "intial amount of contingency time and cost needed", but this is only the intial, and also, what about risks that never reaches the quantitative analysis?
