Changes related to Contract are taken through the Contract Change control board CCCB else it should be Integrated Change contol board for other changes. The aaproved change request would be coming out of performance measurment done, deliverables inspected, quality control, change to any if project constraints (scope, time, cost, quslity, cust sat, risk ect).
So it depends on the question being asked with reference to scenarion described in it.
