A few CAPM questions about phase switching

 Dear All,

I'm looking for a PM guru to help me out on these phases switching stories.
A project is composed of 5 phases. At the very begining I thought these 5 phases were sequential and chronological. But it's obviously not the case.

Questions:

A project always begins by Initiating. Once Initiating is done, we can do Planning. Once planning is done, we can do Executing and Control and Monitoring Phase (both are running at the same time).  Is it correct ?

 

Executing Phase and Control and Monitoring Phase are executed at the very same time. Executing to create the project deliverables. C&M to monitor the project execution. Is it right?

 

- During a project there are several jump from Executing to Closing phase (every time we ask the customer to validate a deliverable, everytime we end a procurement, mid term lesson learned). Is it right ?

  (I found lesson learned both in Close Project or Phase and in Direct and Manage Project Execution, which one is right ?)

 

- In the middle of a project, if there is an important change what should we do?
Perform integrated change control process-> back to Planning Phase to change the Project Management Plan
If the Project Charter do not reflect the actual project anymore, do we go back to the Initiating phase to update the Project Charter and identify again StakeHolder?

Both are right. Lessons learnt are documented throughout the project and during closing phase they are all collated and any final lessons learnt or missing ones can be documented.

Project charter has to reflect actual project, else the project may be terminated or scrapped. There may be changes to project but original charter or objectives has to remain constant.
