The changes Request has to be approved by CCB (Change Control Board ) only, and you can have levels in CCB , which you define in your Project Management Plan, upto what level who all constitutes the CCB.
PM defines what all changes needs CCB approval or needs Change Request, there can be n number of things which PM can change without following Change Management Process, but these things has to be difined and agreed in Project Management Plan. Like its possible in some projects you can update communication management plan without change request.
