In one of the mock tests, this question was asked -
"While performing an audit on a project run by another project manager, a certified PMP, you find that project processes have not been followed, and that company procurement policies have been repeatedly violated. What do you do?
A) Develop a plan to deal with the associated problems.
B) Ask the project manager to explain why these things have happened.
C) Notify the appropriate management immediately.
D) File a complaint with PMI
Correct answer provided was C -
Although per me it should be B as we need to verify the cause and get the facts, only then report it to senior management.. Is that right?

This is the case of violation
This is the case of violation . We dont know what is the level of seriousness of the issue. If it was a small correction or deviation you could have asked the PM for details. Violation is to be reported to higher authorities or as per laid out process.

The key information is: "company policies have been violated". That doesn't sound like "a mistake" to explain, so the first thing to do is inform management.
In other questions you can find something like "a team member told you that a colleague PMP manager is not following procedures..". Usually in this kind of questions the correct answer is investigate further , not report directly.

The audit process is supposed
The audit process is supposed to be a 'structural' process which only draws conclusion on verified facts.
Furthermore, the tone of the question is quite affirmative and it doesn't not say something like 'suspect', 'doubtful', skeptical' etc. about the violation.

Another thought
In any company when you perform audits, the main out put is compliance and non compliance reports.
Atleast in in pervious company they call it NCR (Non - Compliance reports), once the audit is over they are sent to leads for review and subsequent actions are taken as per the process.
Here reporting the managment is similar to sending NCR to the project/process leads.
