Plan Risk Response - updates to Project management plan without change approval

Hi,
I am trying to understand the Plan risk response process, here I find that, since risk response actions/strategies, Project management plan/subsidiery plans are updated. My questions are since this is a change to already approved baselines why these response actions are not performed through change/Perform integraded change control. If my understanding is wrong, please corrrect me. One more thing PMBOK says Response actions are impemented in Monitor and control process (Not in Excecution process???)

Plan Risk Response is in Planning
Hi,
Plan risk responses is a planning proces. Planning processes do not need change requests as they are iterated before the final PM plan is baselined. M&C risks happens in M&C process group. This is not implementing PM plan (execution) but monitoring and controlling the risks (looking out for triggers, finding new risks etc etc)
Hope this makes it clear.
